New Students: If your child is new to St. Patrick, please be sure that a complete immunization record has been forwarded from your previous school.

All Students: Blue emergency procedure cards (one per family) and white emergency medical consent forms are sent home the first week of school. if you have not returned yours, please do so. These need to be filled out completely. All cards must be signed. Only initial the box in front of your child's name if you would want pain relievers administered at school. Also, please forward any changes in information that may occur during the year, such as new addresses or new owrk or cell phone numbers.

Absence: If your child will be absent, please call the Absence Recorder at 419-382-3803 or the school at 419-381-1775, ext. 134 before 9am. All absences must be reported daily unless an Extended Leave Form has been filed. Parents will be phoned if students cannot be accounted for.

Appointments:  If your child has an appointment during school hours, send a note to school stating what time you intend to pick up your child. Your child will meet you in the Nurse's Office where you (or the person designated in your note) will sign out the students. If your child has an appointment and may be late to school, please leave a message on the Absence Recorder at 419-382-3803 or the school number at 419-381-1775, ext. 134. You may even call and leave the message the night before the appointment.

Tardiness:  All students entering the building after 8:30 a.m. must report to the Nurse's Office. By doing so, the student will be taken off the absence list. 

Medication:  On the blue Family Emergency Procedure Card you will note that generic ibuprofen or acetaminophen (Tylenol) can be administered at school. Place initial the box in front of your child's name if you would want pain reliever administered. All other medication must have written consent of the parents and physician. Our fax number is 419-389-1161. 

Nurse Lori's hours are 9-3.  If the nurse's office is closed for any reason, please proceed to the school office. Phone messages can be left any time at 419-381-1775, ext. 134. 

Mrs. Witherell will be in the nurse's office every morning doing attendance from 8:30 to 9am.
